About areola/nipple reduction surgery
It is not uncommon for patients to feel uncomfortable or self-conscious because of the size or shape of their areola.
The patient may feel that the areola is too big or ‘puffy’ and looks out of proportion with the rest of the breast.
There are no medical complications from having large areolae, but reducing them in size can positively affect the patient’s self-esteem and body image.
Enlarged or hypertrophic breast nipples are generally caused by hereditary factors or by breastfeeding, which can stretch a woman’s nipple. Many female patients who are uncomfortable with the change in their nipples after breastfeeding will choose to have this operation.
Although the majority of people who undergo this surgery are women, some men also choose to undergo the procedure because they are uncomfortable with the size and shape of their nipples.
The surgical process
An areola reduction operation removes the excess areolar skin under either local or general anaesthetic, usually lasting less than one hour.
The cosmetic surgeon performs the areola reduction procedure by removing a circular area of the pigmented areola in a quasi-doughnut pattern.
The surrounding skin is then brought inwards in a purse string manner to surround the areola.
Absorbable stitches inside the areola are used to reduce the diameter and generally the surgeon will also use a permanent stitch deep in the areola tissue to stop any widening of the areola following the areola reduction surgery.
Breast nipple reduction surgery aims primarily to shorten the nipple length, while secondarily establishing balance between its diameter size and projection. A nipple reduction is normally done under local anesthetic, while the operation takes about an hour to complete. Nipple reduction cosmetic surgery is usually considered to be an outpatient procedure.
Breast nipple reduction involves removing some of the length or width of the nipple. The type of procedure depends on whether the patient considers their nipple to be too long, too wide, or both.
During a nipple reduction, tiny incisions are made in the patient’s nipple and little pieces of the nipple are removed piece by piece.
To reduce the width of the nipple, a small circle shape is taken out of the middle of the nipple to, in effect, move the nipple in.
Reducing the length of the nipple is a more complicated surgery but the most common process is to simply remove the end of the enlarged nipple.
The skin around the base of the patient’s nipple can also be removed if that is the reason for the prominence of the nipple.
Examples of negligence
Most patients who undergo areola or nipple reductions fully recover from the cosmetic surgery procedure within a couple weeks, provided the surgery is problem free.
Complications from these surgeries are not particularly common, but every surgical procedure has risks and areola or nipple reduction surgeries are not exceptions.
Common problems that can arise during areola or nipple reduction procedures that are caused by cosmetic surgery negligence include:
- Asymmetry of the areolae
- Necrosis or tissue death
- Inappropriate scarring of the areolae or nipples
- Asymmetrical results, meaning the nipples do not match in size or shape following the nipple reduction surgery
- Insufficient nipple tissue removal
- The amount of nipple tissue removed is significantly different to what was agreed between surgeon and patient
- Excessive nipple tissue removal
A bad areola or nipple reduction operation can also result in negative psychological effects related to the patient’s self-esteem and confidence.
